To Register by Mail:
Please attach a photo of your pet to this form (complete body photo with animal facing the camera) and return both copies to the MAAN office. Include your name and pet’s name on back of photo. An ID Tag will be issued and mailed within one week. Please attach the tag to your pet’s collar immediately. If your animal becomes lost, please contact the MAAN office immediately.
